Here you will find information on the programs at the Toronto Central Community Care Access Centre.

Adult Supportive Care 

To support adults with complex care needs self manage their conditions with a focus on promoting and preserving independence in the home and community.

Child & Family 

To support children and their families in achieving optimal growth and development through the provision of community based health services.

Community Independence 

To support seniors and adults to maintain their health and well being so they can age at home and in their communities by fostering self management and linkage to community based resources. 

Hospital Transition 

Focus on Emergency Department (ED)/Alternate Level of Care (ALC) & improving client experience during hospital transitions.  

Information & Referral 

To provide an access point for the public and providing the best possible client experience to callers. This program supports clients with information only requests and helps clients navigate through the health care system.  Referrals are for our services and any other services in the community clients may require access to.

Palliative Care 

To provide quality client-centred care to individuals, who are living with or dying of an advanced illness, and their families and to ease suffering in order to provide the highest quality of life possible throughout the illness trajectory.

Seniors Enhanced Care

To support frail seniors with complex needs & their caregivers to remain at home and in the community as long as possible by providing enhanced support and a range of home and community based service

Acute and Rehab Transitional Program

To support adults with acute or rehabilitative needs to transition to self-care by providing the required CCAC supports along with a range of home and community based services.

Urban Health 

The Urban Health Program delivers services to clients with mental health illnesses and urban health issues. The program currently offers mental health services and supports, transition support to clients at CAMH and services to clients in shelters through our Inner City Access Program. 

FAST Centres

To provide Nursing Care Services to clients who are ambulatory and/or transportable and thus resourcefully allocating in-home nursing services for clients with limited community access.
